The National Rural Health Association recently announced Allevant Solutions as their Preferred Rural Post-Acute and Swing Bed Partner.
Allevant is a joint venture between Mayo Clinic and Select Medical specifically designed for Critical Access Hospitals. Allevant is working in over 40 Critical Access Hospitals. Many more acute care hospitals are looking for high-quality post-acute care pathways for managing patients that are at high risk for readmission and poor outcomes. The program is based on the successful Transitional Post-Acute Care model that was developed in 11 Critical Access Hospitals in Minnesota, Wisconsin, and Iowa by Dr. Mark Lindsay, who serves as Allevant's Medical Director.
Focused on helping patients transition to their highest level of independence, Transitional Care is a program that involves the patient and family, makes available the full resources of the Critical Access Hospital, and focuses on measured outcomes and evidence-based processes to ensure patients get the best care possible. The Transitional Care Program is centered on teamwork, communication, and collaboration. Along with project management, staff training and education, a quality database, and process improvement; Allevant also provides marketing support and referral education to help Critical Access Hospitals grow their Swing Bed volume.
